Asthma Self-Management Programs

The goal of the asthma programs is to help keep your asthma under control. The Suffolk Care Collaborative has partnered with Stony Brook Children’s Hospital, Northwell Health Services and Catholic Health Services to provide home visits to children and young adults in need of support. Lifestyle and wellness education programs can help you learn how to manage your chronic condition. Programs are held in a group setting with others who may be facing the same challenges and feelings. These programs are designed to empower you to overcome obstacles and create healthy habits.

Better Choices, Better Health® – Chronic Disease

Stanford University created the Better Choices, Better Health chronic disease workshop series, also known as the Chronic Disease Self-Management Program (CDSMP). This program can help with the treatment and management of ANY chronic health issue you are facing. This program is offered for FREE at various locations around Suffolk County.

Diabetes Prevention Program

The National Diabetes Prevention Program (DPP) is a lifestyle change program. It is a program to help those who have pre-diabetes or who are at risk for type 2 diabetes, but who do not already have diabetes. This program is offered at various locations in Suffolk County. Additional Diabetes Self-Management Education Programs

The American Diabetes Association (ADA) has many Diabetes Self-Management Education (DSME) programs. Professionals teach programs to individuals or in groups, where you will learn how to manage your chronic condition. A referral from a physician is required.
