The SCC Coalition Partner Participation Agreement is the master services agreement (MSA) for participation. This MSA includes all the key terms of contracting with the SCC. This contract is considered the basic template contract applicable to any SCC Coalition Partner.

We’ve created an SCC Engaged Coalition Partner Participation Manual. The purpose of this document is to provide the Engaged SCC Coalition Partner requirements that must be attained to demonstrate successful completion of Project Requirements to guarantee DSRIP funds flow payment.

How to determine if you’re an engaged SCC Coalition Partner?

To determine if you are an Engaged SCC Coalition Partner, first determine your NYS DOH DSRIP Provider Type classification. Please note, a SCC Coalition Partner may be classified as more than one Provide Type. If this is the case, you will be expected to complete all Project Requirements for all Provider Type classifications assigned to you. Once your provider type is determined, Exhibit B of the SCC Coalition Partner Participation Agreement will describe in which project(s) you are required to participate. The SCC Coalition Partner Participation Manual will outline by project, a set of requirements that you are required to fulfill.

To standardize your submissions, many templates have been created and are referenced herein. Many of these templates are prescribed from the NYS Department of Health other designed by the SCC Project Management Office. Any deviation from the format prescribed could result in a rejection of the information and thus adversely impact the award of achievement values and subsequently earning of the Domain 1-4 DSRIP payments.
