Community Health Associate

Under the direction of the Care Manager and the Social Worker, the Community Health Associate facilitates linkage to community resources so the patient can obtain optimal behavioral health and medical outcomes and remain safe in the community. The Community Health Associate is an instrumental member of a multidisciplinary team, working within the community to provide outreach to patients with complex medical, behavioral health and psychosocial needs.
